How It Works
- Choose a monthly contribution for auto-withdrawal on the day of your choice.
- Your funds are added to your Saltbox Wallet for use on treatments and retail products at your next visit.
- SSA funds never expire and can even be transferred to another patient for a small $20 fee.
- Three-month minimum commitment required. After this initial period, you may cancel at any time without penalty. Early cancellation within the 3-month commitment will incur a $100 fee.

Policies & Terms
- No refunds unless otherwise stated.
- Unused funds may be transferred to another patient for a $20 fee.
- Credit card fees may apply.
- SSA funds never expire and cannot be redeemed for cash.
- In the event of relocation, remaining funds may be used for online purchases.
- Should a medical condition prevent receipt of treatments, a refund (minus credit card fees and reversal costs) will be provided with proper medical documentation.
- Returned or failed automatic payments may incur fees and could affect membership status.
- Membership may be terminated online (when available) or via phone; email requests are not guaranteed.
Definitions
“We,” “us” and “our” mean Saltbox Apothecary and Aesthetic Record, both authorized to process Automatic Bank Payment withdrawals for savings to be used at our establishment. “You,” “your” and “yours” mean the person(s) authorizing the Automatic Bank Payment withdrawals for this membership. “Automatic Bank Payment” means withdrawals automatically deducted from your designated bank account on the scheduled withdrawal dates for the amounts due. “Business day” means Monday through Friday, excluding our company holidays.
Service Provider
You authorize us to use a third party to process the authorized Automatic Bank Payment withdrawals.
Application of Payments
- Funds withdrawn will be applied only to the designated membership account or its replacement.
- Payments made after 6:30 p.m. Central Time may be posted on the next business day.
- You agree to have the funds available in the designated bank account on the payment date, whether or not the date falls on a business day. (Note: It may take 3-5 business days for your bank account to reflect the payment.)
Minimum Commitment and Early Cancellation
- Enrollment in the Saltbox Savings Account (SSA) requires a minimum commitment of 3 consecutive months.
- Early Cancellation Fee: If the account is canceled before completing the 3-month minimum, a $100 early cancellation fee will apply.
- Post-Minimum Cancellation: After the initial 3-month commitment has been fulfilled, the account may be canceled without additional fees.
Payments Not Honored
Automatic Bank Payment withdrawals that are not honored for reasons such as insufficient funds may be resubmitted at our discretion. If we are unable to electronically withdraw the funds from your bank account, any corresponding payment posted in good faith will be reversed from your billing account and a cancellation notice may be issued for the membership attached to the designated billing account.
Removal from Automatic Bank Payment Process
If a payment is not honored, we may remove your billing account from the Automatic Bank Payment method of payment. If this happens, you will need to re-enroll for the Automatic Bank Payment payment method by completing and agreeing to a new Membership Payment Authorization with a valid bank account number.
Returned Payment Charges
We may assess Returned Payment Charges if Automatic Bank Payment withdrawals are returned for reasons such as insufficient funds, closed bank account or revoked authorization.
Excessive Returned Payments or Stopped Drafts
If we receive multiple returned Automatic Bank Payment withdrawals, or multiple stopped drafts, your billing account may become ineligible for the Automatic Bank Payment process until a specified date, set at our discretion.
Termination of Membership
Membership may be terminated at any time. Inactive memberships may use the remaining funds therein but will not qualify for perks of membership. Memberships must be terminated either online (when available) or by contacting us directly via telephone. Emailed requests for termination are not guaranteed.
Transfer of Funds
You may transfer funds from your membership to another patient’s account for a $20 fee.
Refunds
Refunds on contributions will not be given unless otherwise stated in this agreement.
Relocation
In the event of sudden relocation, remaining funds may be used for online purchases. Saltbox staff will assist in this process.
Inability to Receive Treatments
In the event of a change in medical status that renders you unable to seek services for an extended period of time, Saltbox Apothecary will refund accumulated funds in the form of a mailed check minus 15% to account for credit card fees and reversal efforts.
